A short first meeting for clients, families and Allied Health Assistants (AHAs) to get to know each other and agree on how to work together.

Your Allied Health Professional (AHP) joins later, at the Team Meeting.

Before

In short: Complete the COVID-19 pre-session check before meeting in person.

COVID-19 pre-session check

Before every in-person session at a client's home, the Allied Health Assistant does a quick health check. It can be a 5-minute phone call, an SMS or an email.

In the last 7 days, has anyone in the household:

  • Tested positive, or presumptively positive, for COVID-19, or been identified as a possible carrier of COVID-19 or a similar communicable illness?
  • Had symptoms commonly associated with COVID-19, such as a cough, fever, difficulty breathing, muscle aches and pains or general exhaustion?
  • Been in direct contact with, or close to, anyone known to have COVID-19 or identified as a possible carrier?
  • Finished an isolation period after testing positive or being a close contact?

The client or their parent or guardian then asks the Allied Health Assistant the same questions.

During

In short: get to know each other and discuss if and how to work together.

A 30-60 minute meeting between the AHA and the person seeking support, and their family. Each of you learns more about the other, then you agree whether and how to work together.

No therapy takes place during a Meet & Greet. It is not charged to the client, and the AHA is not paid for it. Every session after the Meet & Greet is a paid session.

Work through these questions together. They cover goals, the role and availability, so there are no surprises later.

For the client

The person seeking support, or their family

  1. What goals are you hoping to work towards?
  2. Do you have short-term goals, for the next 6 months?
  3. Do you have long-term goals, over the next few years?
  4. Where will sessions take place: at home or in the community?
  5. Do you have any holidays planned?
  6. Will you be moving suburbs any time soon?

For the AHA

To talk through with the client and family

  1. Along with therapy, will you take on extra tasks, like preparing meals, transport or community activities?
  2. Will there be personal care needs during sessions, such as dressing or toileting?
  3. If working with a child, will you be expected to work without a parent present? Are you comfortable with that?Fora expects a few sessions and good rapport before any unsupervised sessions.
  4. Which days and times are you available? Will this change in the holidays?
  5. Will you be changing courses or universities? How often does your timetable change?
  6. Do you have placements this semester or next? If so, could evening or weekend sessions work in the meantime?

What
A paid meeting between the client, their AHP and the new AHA. The AHP identifies suitable tasks for the AHA, so everyone is aligned on the work.
Why
The AHA fills out a Team Meeting Form that records the tasks delegated to them.
How
The AHA contacts the client or their family to find a time.
Who attends
Ideally the AHA, the AHP and the client or their family. At minimum, the AHA and the AHP.
Format
Ideally in person, or by video or phone call.

What you need to do

Client or family

  • Answer the pre-session safety questions. You can ask the AHA the same questions.
  • Discuss goals, where sessions will take place and any planned changes.
  • Tell Fora how the meeting went and whether you would like to proceed.

AHA

  • Conduct the pre-session safety check.
  • Discuss the tasks, personal care needs, supervision and your availability.
  • Tell Fora how it went. If both parties proceed, contact the client or family to arrange the Team Meeting.
